Important safety rules for tension rings:

How do I know which tension ring size I need, and what does the size number mean?

The tension ring size corresponds to the internal diameter of the ring — the opening through which the penis passes when the ring is transferred from the pump cylinder to the base of the penis after the erection is achieved. A ring that is too large will not provide sufficient constriction to maintain the erection after the vacuum is released. A ring that is too small may be uncomfortable, difficult to transfer, or overly constrictive. The five rings included in the standard Vacurect OTC kit (sizes 1 through 5) are the most commonly used sizes and cover most users. If you have been using your Vacurect and consistently find that the smallest ring in the kit (size 1/old #28) is the right fit or close to it, you may want to try size 2 (old #26) as a comparison. If you find the largest ring in the kit (size 5/old #20) is too small, sizes 6 (old #19) or larger are available. For users starting VED therapy for the first time and trying to determine their ring size without prior experience, the user manual provides guidance on fitting, and your prescribing physician or urologist can advise on the starting size based on your anatomy. Why is there a 30-minute maximum wear time for the tension ring and what happens if it is worn longer?

The tension ring functions by maintaining a controlled constriction at the base of the penis after the vacuum is released — the constriction traps blood in the erectile tissue to maintain the erection. This is a form of temporary penile constriction. Like any applied constriction to a body part, prolonged uninterrupted restriction of blood flow and venous return creates a risk of tissue injury. At 30 minutes of continuous constriction, the risk of tissue ischemia (insufficient oxygen delivery to tissue), nerve compression, and bruising begins to increase meaningfully. Beyond 30 minutes, the risk of more serious complications — including petechial bruising from venous engorgement, numbness from nerve compression, and in cases of excessively long wear, more significant circulatory compromise — increases substantially. The 30-minute limit is an industry-wide standard for all VED constriction rings, not specific to the Vacurect design. It is stated in the Vacurect user manual (pages 2 and 3) as a specific safety guideline. Users should remove the ring at the 30-minute mark regardless of whether the erection has fully subsided. If erections are consistently not maintained for an adequate duration within this 30-minute window, discuss therapy protocol, ring sizing, and any contributing clinical factors with your prescribing physician rather than extending wear time beyond the safe limit.

Why can't I use silicone oil or petroleum-based lubricant on the tension rings?

Vacurect tension rings are made of a latex or latex-blend rubber material. Silicone-based oils and petroleum-based products (petroleum jelly, mineral oil, baby oil, and oil-based massage products) interact chemically with natural rubber and latex by causing the polymer chains in the rubber to swell and plasticize — a process that softens the ring, reduces its elastic recovery, and ultimately causes it to lose its constricting tension and structural integrity over time. A ring that has been repeatedly coated with silicone oil or petroleum products will become overly soft, may not maintain adequate constriction during use, and is more likely to tear. This is also why silicone oil — the Vacurect pump maintenance lubricant used on the pump's O-rings — must never be applied to the tension rings even though it is the same product used elsewhere in the VED system. Water-based personal lubricants do not interact with latex chemistry in this way: they are latex-compatible and safe for use on the tension rings. Always use the Vacurect Water-Based Personal Lubricant (or another water-based personal lubricant) on the ring before each use, and keep silicone oil strictly limited to the pump's O-rings where it is specified.
